Private vs PSU Group Health Insurance: Which Should Your Company Choose?
Private insurers offer faster claims, stronger digital tools, and flexible products. PSU insurers cost less and offer GIPSA rate benefits. This guide helps you decide based on your company type, workforce, and priorities.
Understanding the Difference
What are PSU (Government) Health Insurers?
PSU (Public Sector Undertaking) health insurers are government-owned general insurance companies regulated by the Ministry of Finance and IRDAI. The four major PSU general insurers offering group health insurance in India are New India Assurance, Oriental Insurance, United India Insurance, and National Insurance Company.
PSU insurers have been operating for over 50 years and are trusted by government bodies, PSU organisations, and manufacturing companies. Their main advantage is lower premium and access to GIPSA-negotiated hospital rates that reduce out-of-pocket costs for employees.
Private Insurers
What are Private Health Insurers?
Private health insurers in India include both standalone health insurers (Star Health, Niva Bupa, Aditya Birla Health, ManipalCigna) and private general insurers (HDFC ERGO, ICICI Lombard, Bajaj Allianz, Tata AIG). These are privately owned, IRDAI-regulated companies.
Private insurers typically offer faster cashless claim processing (1–4 hours pre-authorisation), stronger digital self-service portals, wellness program integrations, and more product flexibility. Their premiums are higher, but the service quality and employee experience are significantly better.

ICR and What it Means for Your Renewal Premium
The Incurred Claim Ratio (ICR) is the percentage of premium paid out as claims. PSU insurers have ICRs of 100–103%, meaning they pay out more than they collect. This seems counterintuitive to higher renewal loading, but PSU pricing is partially cross-subsidised by government operations.
For your group policy specifically, renewal loading depends on your group's claim experience, not the insurer's overall ICR. A group with high claims will see loading at renewal regardless of whether you are with a private or PSU insurer.
However, private insurers typically have more sophisticated actuarial tools to price and model group renewals, which can mean more predictable (and sometimes lower) loading for low-claim groups. PSU renewals can be more volatile, especially when office-level branch decisions vary.
PSU Advantage
What is GIPSA and Why Does it Matter?
GIPSA (General Insurance Public Sector Association) is a consortium of India's four PSU general insurers (New India, Oriental, United India, National Insurance). GIPSA negotiates standardised treatment package rates with empanelled hospitals across India.
Fixed Package Rates
Hospitals accept GIPSA-defined amounts per procedure, preventing bill inflation.
Lower Out-of-Pocket
Employees in GIPSA-empanelled hospitals often face less balance billing.
Wider Tier-3 Reach
GIPSA hospitals include many Tier-3 and rural hospitals that private networks may miss.
Note: GIPSA rates are only available with PSU insurer policies. Private insurer policies use individually negotiated hospital tariffs, which can be higher but are also more current and regularly updated.
When to Choose Private Insurers
- Startup, tech company, or any business where employee experience matters
- You need fast cashless pre-authorisation (1–4 hours vs 4–24 hours)

- You want OPD cover, wellness programs, mental health add-ons
- Your workforce is in Tier-1 or Tier-2 cities with modern hospital preferences
- You value dedicated relationship managers and transparent renewal discussions
When to Choose PSU Insurers
- Government body, PSU organisation, or public-sector entity with procurement guidelines
- Your primary goal is lowest possible premium for a basic benefit structure
- Your workforce is in Tier-3 cities or rural areas well-served by GIPSA hospitals
- You do not need digital tools, HRMS integration, or wellness programs
- Your employees are used to branch-based claim processes (e.g., legacy manufacturing)
- You have contractual or compliance reasons to prefer government-backed insurers
